OCI - Name Change Affidavit

I changed my name by court order. Do I just need to write a letter saying I changed my name and get it notarized. And send it in along with a copy of my court name change decree?

Update: Found a form from vfsglobal used for passport application. Filled it out got it notarized and sending it in along with a copy of the court decree. Will update of it is accepted.

Did you receive any update ?
I've similar issue where "bahen" was added behind my wife's name and We changed spelling and removed bahen in my wife's name and I have the "Petition for Name Change" from USCIS.
I didn't have any issues while Renouncing the passport, but just confused why they make it so hard, but please let me know if you receive any update.
